Pain after Root Canal Treatment or RCT

There can be several reasons why one can still have Pain After RCT or Root Canal Treatment. Some of the most common once are listed below:
1. The instrument that is used to clean the root canal space might get fractured within the root canal space thereby not allow complete cleaning of root canal space. This can be due to over use of Instruments meant to be discarded after every single RCT case. Appling excessive pressure while cleaning, calcified canals or severely curved root canal etc. may also lead instrument separation.
2. Broken Instrument if present beyond the tooth into the bone will have more severe repercussions like swelling, pain or pus formation.
3. Pain can also arise if there is a over filling beyond the tooth into the bone.
4. Incomplete filling of the root canal space can also result in accumulation of bacteria in the remaining root canal thereby leading to pain or abscess.
5. RCT is a complicated procedure specially in the posterior teeth because it has more number of root canals and the dentist should be able to locate all the canals. There can be up to 6 canals in the molars although it varies from person to person.
6. Crowns/ Capping having marginal Discrepancies i.e if the fabricated crown margins are placed beyond the tooth and over the gums. It can lead to food accumalation around the crown margin which later causes inflammation and pain at the gum margins
7. A perfectly done Root Canal Treatment may also end up in pain if the used instrument is infected and bacteria is transferred from one patient to the another.

Treatment Options
(A) Root Canal Treatment should be repeated again which is also called as Re-RCT Treatment. It’s normally carried out by the root canal specialist who is also called as Endodontist.
(B) EXTRACTION /Tooth Removal
